Community Partnership Initiative

What is it?

The CDM  Community Partnership Initiative (CPI) is a new and innovative program created to meet the increasingly complex needs of our area’s most vulnerable citizens.  The CPI was developed to reach beyond traditional in-home care services, and solve an unmet need in the community.

Every day, in the course of providing in home care through Medicare, Medicaid, and other state and federal funds, CDM Services encounters needs in our community that cannot be met through traditional tax funded sources.  These government funding sources, and the rules regarding financial eligibility, are becoming more restricted all the time.  And few resources are available for clients that do not meet state and federal guidelines.  Medicaid clients must pay for a portion of their services, leaving them with less than the federal poverty level for basic living expenses.  Those with disabilities have unique individual needs that are not met by government programs.  These people are going without eyeglasses, hearing aids, canes, walkers, medical equipment, and medications.

Clearly, the current system is stretched to capacity and community support is the only way that these needs can be fully met.  The CPI was developed so that CDM can take the lead in filling this gap through service scholarships, and expanded services.  This initiative will serve to identify and cultivate community relationships, and partnerships, and to develop long term financial support to address these unmet needs in your community.

Where will the funding come from?

The start up and initial program costs for the CPI have been funded by a generous grant from the M.J. Murdock Charitable Trust.  But, we still need your support!  The unmet needs of vulnerable people in your community can only be met through donations from people like you.  The CPI will work to develop financial support through partnerships with companies, organizations, foundations, and individual donors.

 

Where will your money go?

Your donation will directly help a person in your community by providing service scholarships, and expanded services.  These volunteer-based, community programs will work to address the need for medical equipment, medications, therapy, and equipment for the disabled, transportation to doctor’s appointments, home repairs, and quality of life improvements.

 

How can you make a gift to the Community Partnership Initiative?
